Scholarship Application Forms

Applications for scholarship grants from the U.S. JCI Senate Foundation must come from high school students who are U.S. Citizens graduating from U.S. accredited schools. This may include state approved home school and state approved GED programs, cyber schools, and overseas U.S. approved schools.

The Foundation is a fully qualified, IRS approved 501(c)(3) foundation. Our IRS Tax ID Number is 38-2680101. All contributions to the Foundation are deductible for federal income and estate tax purposes.

From September to November of each year, each JCI Senate state organization (or foundation) distributes to schools and boards of education throughout their state the necessary application forms for determination of the recipients of  scholarship grants. The deadline for return of the completed application forms to the state chairperson is at his or her individual discretion, and can vary from as early as December 1st to as late as the following January 15th. The deadline is shown on each state's application cover page.

From these files, select the application form for the state in which the student applicant resides. Other than inserting the requested information, alterations to the form could result in the applicant being disqualified.

Beginning in 2010, applications are available as PDF forms only. To open and print them, you will need a fairly recent version of the free Adobe Reader (version 7 or above).

If your school's network firewall will not allow you to download a JCI Senate state's scholarship grant application from a web page, you should be able to use FTP (File Transfer Protocol) instead. Consult with your institution's IT staff on how to install and use an FTP client on your computer.

Again, please note: It is vital that you obtain the scholarship grant application for the state IN WHICH THE APPLICANT RESIDES!! Where the applicant plans to attend college or vocational school is irrelevant.
 

Applications for the 2010-2011 school year will be removed from this list once their submission deadlines have passed.
